The Patient Portal makes your medical information more accessible to help you live a healthier life.
- The Patient Portal is a secure, confidential, and easy-to-use website that gives patients and their families 24-hour access to their medical records.
- It uses the latest encryption technology to deliver secure communication between patients and providers.
By signing up and enrolling with the Patient Portal, you will be able to:
- Schedule appointments and recieve appointment reminders
- Access important health information from your medical records including immunizations
- View lab test results
In the next few months, we’ll be adding more options allowing you to:
- See the patient plans from your office
- Communicate with your provider

How do I retrieve my user name?
Click on the need help with user name and password link and follow the instructions. A link to reset your password will be emailed.
How do I retrieve my password?
Reset the password by entering the user name and answering the security question. A link to reset your password will be emailed.
Your doctor’s office also can help you reset the password by providing a link and a password reset token.
Can I use the Patient Portal on my mobile device?
Yes, the NextGen Patient Portal is now available for mobile devices and tablets.
There is no special app to download, just open your enrollment email and click Patient Portal link or enter the link in your mobile browser to access the mobile site.
Can I book appointments from the Patient Portal?
You can book appointments online using NextGen Patient Portal as well as send an appointment request to the practice if you are unable to book an appointment.
In addition, you can reschedule or cancel appointments through the patient portal.
Can I see lab results on NextGen Patient Portal?
Documents and lab results are automatically uploaded to NextGen Patient Portal once your provider has reviewed them.
You will recieve an email notification letting you know when a document is available in the inbox.
Can I add care managers and dependents?
You can add a care manager to manage the selected patient’s NextGen Patient Portal account, add a dependent for the selected patient to manage at the time of enrollment or at the request of the patient.
How do I Sign Up?
You can sign up and register for the patient portal by giving your provider your name and email address at your next visit.
In turn, you will receive an instruction sheet that includes an activation code, known as an enrollment token.
The enrollment token will allow you to log in to the system and create your own private user name and password.
Who can Sign Up?
Patients and their legal guardians can sign up for the patient portal in accordance with the terms and conditions of the website.
Just tell your provider who would like to enroll and you are on your way to connecting with your provider.

The NextGen Patient Portal is designed to protect your privacy and keep communication and medical records secure.
If you choose to use the Patient Portal, your name and email address will be treated with the same care and privacy given to your medical records.
You will receive notices to your personal email account when there is important information awaiting you in your Patient Portal account.
