New Patients

Welcome to Desert Senita! If you are a new patient, you will be required to fill out the following forms:

· Demographics (download)
· HIPAA Notice of Privacy Practices (download) (link)
· Patient’s Rights & Responsibilities (download) (link)

As a new patient we will also require a copy of your up-to-date insurance cards along with the following information:

· Home Phone
· Cell Phone
· Physical Address
· Insurance
· Emergency Contact
· Name and number of parent or guardian if the patient is a minor

 

Appointments

Please be aware that appointments are scheduled in advance. Same-day appointments are available but are limited only to patients suffering acute illness. This means that physicals and medication refills are not eligible for same-day appointments. If you do require a same-day appointment, you shall be transferred to our nursing staff who will schedule your same-day appointment.

 

Making an Appointment

Appointments are made over the phone ((520) 387-5651) or in person during normal business hours. When making an appointment we will require the following information:

· The name of your Primary Care Provider (PCP)
· The nature of your visit

Our front office staff will then provide the available dates and times for appointment slots for you to choose from. Once an appointment slot has been chosen, our staff will verify your check-in time and co-pay required. Your insurance will also be verified on the date the appointment is scheduled and again on the date of service.

Desert Senita is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Federal Tort Claims Act (FTCA) Public Health Service employee under 42 U.S.C. 233(g)-(n).
